Combined Ship's Log Book format is intended for ships of GT less than 500 (e.g., fishing vessels, harbour fleet). Combined Ship's Log Book is of the form where Engine Log Book and Ship's Log Book are merged.

Garbage Record Book Part I

Garbage Record Book Part I (new version in force from 01.03.2018). This form of the Garbage Record Book shall be used:
  • on every ship of 400 gross tonnage and above;
  • on every ship which are certified to carry 15 or more persons engaged in voyages to ports or offshore terminals under the jurisdiction of another Party to the Convention;
  • on every fixed or floating platforms.
Current Garbage Record Book Part I consists of two separate parts:
  • Part A - Record of Garbage Discharges;
  • Part B - Exceptional Discharge or Loss of Garbage.

Garbage Record Book Part II

Garbage Record Book Part II (new version in force from 01.03.2018.).
This form of the Garbage Record Book shall be used on ships of 400 gross tonnage and above carrying solid bulk cargoes, other than grain, as defined in regulation VI/1-1.2 of the SOLAS Convention, 1974, as amended.

GMDSS Radio Log Book

GMDSS Radio Log Book shall be used on following ships engaged in international voyages:
  • Cargo ships of 300 GT and above
  • All passenger ships
The following records in GMDSS Radio Log Book shall be made:
  • Summary of distress, urgency and safety radio communications
  • Important incidents relating to radio services
  • The position of the ship at least once a day
  • A summary of the condition of the radio equipment and tests performed thereof.

Oil Record Book (Part 1)

Oil Record Book Part 1 (Machinery Space Operations) shall be used on every oil tanker of 150 GT and above and every ship of 400 GT and above other than oil tanker.
